This letter is sent to you as a formal reprimand regarding your lack of discipline at the Company. This written warning is being issued to you for certain unacceptable personal conduct.

For your review, enclosed is a copy of the company’s Disciplinary Policy and Procedure. To assist me in the investigation process, I would like to set up a meeting with you wherein we can discuss the details of the allegations warranted for such disciplinary actions.
